Foundation Medical Partners’ outpatient practices are open for business and are offering alternative ways, such as telehealth visits, to care for you during this time. In addition, all online scheduling is suspended indefinitely. Please call your provider with any questions or to schedule an appointment, and we would be happy to accommodate your needs.

Cancellations and Late Policy

We ask for a 24-hour cancellation notice in order to accommodate other patients. You may be asked to reschedule your appointment if you arrive more than 15 minutes after your scheduled appointment time.

We know that your appointment with your doctor is important to you, and it can be most effective if you are prepared for it. This includes having the information your doctor will need from you as well as being able to gather the information that you need from your doctor.
